An Autopsy case of cyst of Sella Turcica


Abstract
Fortunately, the authors, autopsed a 28-year-old pregnant woman who died suddenly after endometrial curettage under the diagnosis of toxemia and found a large cyst of the sella turcica.
She had lateral strabismus of the left eye for a few year, About one month before operation. she develped occasional headache and generalized weakness and then they become gradually worse. The physical -and laboratory examinations showed no abnormalities except for the slight increase of blood pressure,
Grossly, the cyst of the sella - turcica measured about 3cm in diameter and contained ¢¥about 10cc of clear fluid. The cyst occupied the entire -sella and extended posteriorly to the pontine cisterna and -the upper portion of the pons, anteriorly to the optic chiasm and chiasmic and interpeduncular cisterna and upwardly pars tuberalis. The left optic nerve thinned by the cystic mass but the right one was normal. The clinoid processes of the sella turcica were flattened, a pressure effect of the cyst. The fossa of the sella turcica was slightly enlarged. However, the pituitary gland situated in the fossa seemed to be normal in size. The external appearance of the brain was not remarkable. On the coronal sections of the brain all the ventricules were moderately dilated and filled with large amount of clear cerebrespinal fluid. The adjacent brain tissue of the ventricules became somewhat atrophic.
Microscopically, the cyst wall consisted of dense fibrous connective tissue partially lined with a single layer of flattened, cuboidal or columar epithelia. Between the cyst wall and the capsule of the hypoph¡þysis were noted hemorrhagic foci and a mild infiltrate of small round cells and macrophages. The eosin¡þophilic cells of the anterior lobe increased slightly in mumber. Between the anterior and posterior lobe were also observed a few small cystic cavities which were filled with amorphous, eosinophilic substance and were lined with a single layer of cuboidal and columnar epithelium.
The heart was moderately enlarged and hypertrophic. The spleen also was moderately enlarged and chronically congestive. The other oragans were not remarkable at gross and microscopicy levels.
